why in news :

  • With an aim to reach out to the maximum number of people particularly from the remote areas, the Government has plans to launch tele-consultation facility ‘e-Sanjeevani’ at 1 lakh Ayushman Bharat-Health and Wellness Centres (AB-HWCs) .
  • This was stated by Union Health Minister Mansukh Mandaviya after chairing a virtual meeting on Tuesday with health ministers and senior officials of all the states and union territories to review the operationalisation status of AB-HWCs and tele-consultation services.
  • “On the occasion of the 4th anniversary of Ayushman Bharat Health and Wellness Centres, ‘e-Sanjeevani Tele-consultation’ facility will be launched at 1 lakh centres on April 16.
  • These centres are realising PM @NarendraModi ji’s resolve of ‘Ayushman Bharat’,” Mandaviya said in a tweet in Hindi on Friday.
  • As many as 1,17,440 AB-HWCs had been operationalised across the country by the end of March against the target of 1.1 lakh, according to the Health Ministry.
  • To ensure wider outreach of telemedicine services, eSanjeevani application has also been integrated with 3.74 lakh Common Service Centers (CSCs) thereby facilitating access to equitable healthcare in the remotest areas of the country.

  • Under ECRP-II, financial assistance has been provided to all States/UTs for strengthening IT infrastructure for implementation of telemedicine services.

National Teleconsultation Service :

  • National Teleconsultation Service of Ministry of Health and Family Welfare is first of its kind onlineOPD service offered by a country government to its citizens.
  • National Teleconsultation Service aims to provide healthcare services to patients in their homes. Safe & structured video based clinical consultations between a doctor in a hospital and a patient in the confines of his home are being enabled.
  • Under e-Sanjeevani platform, 56,994 Doctors have been registered to provide teleconsultation services. Till, 31st January, 2022, around 2.33 Cr consultations have been conducted through eSanjeevani platform, as per the data available from the Ministry.
  • To ensure patient data privacy and security, required firewalls which monitor and control the access to the application is enabled. Only valid tokens are enabled to ensure authorized access and the data is stored in an encrypted form duly using industry-standard algorithms,  a senior official said
  • Presently,  eSanjeevani OPD (Patient to Doctor Teleconsultation services) has been rolled out in 25 States/UT while  eSanjeevani HWC (Doctor to Doctor Teleconsultation services) has been rolled out in 20 States/UT.

eSanjeevaniOPD :

  • eSanjeevaniOPD – Stay Home OPD has been developed by Centre for Development of Advanced Computing (C-DAC) in Mohali.
  • Salient features of this citizen friendly web-based National Teleconsultation Service (eSanjeevaniOPD) are:
  1. Patient registration
  2. Token Generation
  3. Queue Management
  4. Audio-Video Consultation with a Doctor
  5. ePrescription
  6. SMS/Email Notifications
  7. Serviced by State’s Doctors
  8. Free Service
  9. Fully configurable (no. of daily slots, no. of doctors/clinics, waiting room slots, consultation time limit etc).
  • eSanjeevaniOPD is based on eSanjeevani – Government of India’s flagship telemedicine technology developed by Centre for Development of Advanced Computing (Mohali). eSanjeevani – a doctor to doctor telemedicine system is being deployed nationally for the Miinistry of Health & Family Welfare at 155,000 Health and Wellness Centres under Ayushman Bharat Scheme of Government of India.

Other provisions:

  1. eSanjeevaniOPD is yet a web-application, however, it’s development has been based on responsive web design approach. Hence eSanjeevaniOPD is expected to respond to the user’s behaviour and environment based on screen-size, platform and orientation. It is possible to use eSanjeevaniOPD on large screen tablets and smartphones.
  2. For a smooth full-motion video consultation experience internet speed of at least 1Mbps is recommended.
  3. Once registered patient’s name, age, gender, patient ID cannot be altered, however, patient can update email / mobile number and address
  4. Every patient is assigned a unique 16 digit patient ID, it is generated using Luhn Algorithm (mod 10 algorithm).
  5. Every consultation will be assigned a unique consultation number.
  6. Patients residing in a particular State will get connected with one of doctors on the panel of doctors set up by the corresponding State’s health department.
  7. If (at xi) patient misses to click “CALL NOW” in 120 seconds, eSanjeevaniOPD will move this patient few slots lower in waiting room queue and the patient next in queue will take the turn.
  8. All tokens (used & unused) will expire at the end of the day.
  9. For using the system again the user will need to follow same steps, registration form will show up auto filled with details existing in eSanjeevaniOPD, hence the user will upload health records if any and then generate token.

Ayushman Bharat – Health and Wellness Centres

  • Ayushman Bharat (AB) is an attempt to move from a selective approach to health care to deliver comprehensive range of services spanning preventive, promotive, curative, rehabilitative and palliative care. It has two components which are complementary to each other.
  • Under its first component, 1,50,000 Health & Wellness Centres (HWCs) will be created to deliver Comprehensive Primary Health Care, that is universal and free to users, with a focus on wellness and the delivery of an expanded range of services closer to the community.
  • The second component is the Pradhan Mantri Jan Arogya Yojana (PM-JAY) which provides health insurance cover of Rs. 5 lakhs per year to over 10 crore poor and vulnerable families for seeking secondary and tertiary care.
  • HWC are envisaged to deliver expanded range services that go beyond Maternal and child health care services to include care for non -communicable diseases, palliative and rehabilitative care, Oral, Eye and ENT care, mental health and first level care for emergencies and trauma , including free essential drugs and diagnostic services
